Welcome Graduate Students! Below are links to helpful guides, handbooks, and forms that will help you meet program requirements.
Handbooks and Forms
New Student Information Guide. In the guide you will find information about housing, Madison, orientation, benefits, enrollment, etc.
Graduate Handbook. In this handbook, you will find information on the policies, requirements, procedures, resources, and norms for graduate students in the EEB program.
Joint Degree Application form. This form must be completed for students intending to add EEB as an additional graduate program.
Annual Progress Report form. This form must be completed annually by all EEB PhD students at UW-Madison.
Course Certification form. This form must be completed in the first year by all EEB PhD students at UW-Madison.
Course Substitution Request form. This form is to be used to change a course on an already approved certification form.
External Committee Member Request form. This form is to be used to approve a non-UW-Madison faculty member or non-faculty member to a graduate committee.
Warrant Request form. This form should be submitted at least five weeks before your preliminary exam or final defense.
Graduate School
